The Trapezist is Misery Town’s fight-or-avoid boss. Choose based on the achievement you want and whether you brought the scalpel.

Keep a save before the encounter if you intend to collect both outcomes without replaying the full area.
If you do not have the scalpel
Return to the Whack-a-Clown puzzle, complete its film-and-color solution, and collect the scalpel. The scalpel item page connects that reward to the operation and the Trapezist rope. Starting the normal fight without this preparation puts you on the Fatal Performance branch.
