Where to get the scalpel

The scalpel is obtained from Misery Town’s Whack-a-Clown puzzle and has two important confirmed uses. Which one matters depends on whether you are solving the red-ball chain or preparing the Trapezist avoidance route.

Get the scalpel

Use the theater films to identify the four clowns, then knock out:

• purple;
• pink;
• red; and
• yellow.

Completing that board gives the scalpel. Use the Whack-a-Clown solution if the clue or color order is still blocking you.

Use 1: operation mini-game

Bring the scalpel to the operation sequence. Guide its tip through the path; the rest of the sprite can overlap the boundary as long as the tip stays clear. Finishing the operation produces the red ball.

Use 2: Cut The Act

Keep the scalpel before the Trapezist encounter. Instead of entering the projectile battle, use it on her support rope to complete the avoidance outcome.

Plan before the encounter

The scalpel links a puzzle reward to both an item task and a boss choice. Do not discard it after the operation if you still want the Trapezist avoidance achievement.
