The Freelance Surgeon is a surreal medical figure associated with some of Endacopia’s most unsettling operation imagery. For players, the useful way to understand the character is through the tasks and spaces where that imagery returns.

Gameplay connections
The Surgeon’s theme connects most clearly to Misery Town:
- the theater and clown chain leads to the scalpel;
- the scalpel is used in the operation mini-game;
- completing that operation produces the red ball; and
- the same scalpel can change the Trapezist encounter.
These are separate tasks that share a medical visual language. The Surgeon’s presence helps connect them, but it does not mean every creature or operation in Endacopia should automatically be attributed to him.
What to do when you reach the related puzzles
Start with Whack-a-Clown if you do not have the scalpel. Continue with the red ball operation guide if the mini-game is blocking you.
Keep the scalpel afterward if you want the Trapezist avoidance outcome. The scalpel item guide lists both confirmed uses.
